Completing the form:
This form must be completed in full for each attendee (students and adults) attending any Georgia Thespians event.

Handwritten forms will not be accepted.

All forms must photocopy clearly.

Do not submit forms from previous years.

Signing the form:

• Forms for student attendees will be signed by the student's parent or legal guardian.
• Forms for adult attendees will be signed by the adult attendee themselves.
• Eighteen-year old (or older) students are not allowed to sign forms for themselves.

All information must be provided in order to complete this form.
